Shopify can serve the same product at more than one path depending on how it was reached, and a store with several collections multiplies that. If an agency cannot tell you how they handle it, they have not worked on Shopify at scale.

Will they touch the theme, or only send recommendations?

A lot of Shopify SEO work is template-level: how collection pages are built, what internal links the theme generates, where metadata comes from. An agency that will not edit the theme is handing that work back to you.
